A personal or professional care team might have full confidence that the individual they support is safe while in their care, but it can be financially and physically taxing to provide for a person with an intellectual disability or dementia around the clock. CreateAbility’s solutions can provide not only peace of mind but reduce the financial burden of employing a full-time staff member that an individual might require. Contact us today to see how our solutions can meet your needs.
SmartMed
With the right medication, people can enjoy lives with less pain, fewer symptoms of their condition, and more freedom, but they are only effective if taken correctly. People with intellectual disabilities or dementia may lack the skills or memory to take their medications properly.
SmartMed improves medication adherence for those who need extra support and is ideal for remote medication management in a home or facility. It provides an affordable option to confirm that the right medication and dosage are dispensed to the correct individual at the necessary time. SmartMed can:
- Remind an individual when it is time to take medication
- Dispense pills at the proper time
- Alert caregivers and family members when the person does not take their medication
- Record when medications are accessed
SmartMed also interfaces with CreateAbility’s care coordination software BEAM to keep everyone informed of caregiving schedules and issues.
Vital Sign Monitoring
With some chronic diseases, it is important to keep an eye on certain vital signs because they can act as an early warning signal that a problem might occur. Measuring vital signs can be confusing sometimes, so CreateAbility’s Vital Sign Monitoring system guides the patient through each step of processes such as proper positioning of blood pressure cuffs, taking a blood sample, and reading reagent strips. This is ideal for those who are managing COPD, congestive heart failure (CHF), and diabetes. The appropriate vital sign monitoring devices can be added to any of the Independence Family systems.
CreateAbility can monitor:
FallStop
A fall can result in a serious injury and severely damage an individual’s health, however, some falls are preventable. CreateAbility’s FallStop works in a home or facility to dramatically reduce the risk of falls. FallStop uses a variety of technologies that can be customized to the patient, such as: special sensors on chairs, beds, and walkers that detect if the person is attempting to walk un-assisted.
FallStop is enhanced when combined with fall assessments on BEAM, SmartMed and Independence Keeper so that it can get a fuller understanding of the individual’s medication adherence and activities of daily living. Fall Stop Plus adds the power of visual pattern recognition to predict or detect fall situations. Fall Stop Plus’ fast analytics detect an unsteady gait, a spiral pattern or swaying when attempting to stand unassisted, then immediately alert staff or nearest responder as it respectfully coaches the individual to wait for assistance.
CareTrak
Elopement, Sundowners, or simply wandering without proper supervision, could be a problem for person aging-in-place. The older adult might become confused or agitated and may be more likely to wander away from their home. With CareTrak watching over an older adult, they can continue aging-in-place longer, from wherever they call home, without compromising their safety.
CareTrak detects an elopement attempt and immediately prompts the individual with distraction techniques while it silently alerts the nearest responder. Distractions include reminding them that they might miss getting a phone call from a loved one or that their favorite TV show is about to come on. An optional component further discourages them from their plans by enabling a sprinkler, strategically positioned at the front door, to automatically turn on.
